Insurance Verification Call Checklist: General & Family Dental

Purpose: Confirm each patient's dental benefits before the visit so we catch frequency limits, waiting periods, and maxed-out benefits ahead of time, not at checkout.

How to use this

  • When: Verify 2–3 business days before the appointment. Re-verify at the new plan year, after any plan or employer change, and before high-cost treatment (crowns, root canals, implants, surgical extractions).
  • Portal first: Check the payer portal or Dentrix eligibility before calling. Call only for what the portal doesn't show.
  • Target call length: 10–15 minutes. Have the questions open and the codes ready.
  • Always write down the rep's name and the call reference number. No reference number means the verification isn't finished.
  • Paste the Call Log (Section 5) into the patient's Dentrix insurance notes the same day.

1. Before You Dial: Prep Checklist

Have in hand:

  • Patient name, DOB
  • Subscriber name, DOB, and relationship to patient (self / spouse / child)
  • Member ID and group number (or employer name)
  • Payer name and the phone number on the back of the patient's card
  • Practice NPI (Type 2 group), Tax ID, and the rendering dentist's NPI
  • Planned CDT codes and tooth numbers, for example:
    • Exams: D0120, D0150, D0140
    • X-rays: D0274/D0272 (bitewings), D0330 (pano), D0210 (FMX), D0220 (PA)
    • Cleanings: D1110 (adult), D1120 (child), D1206/D1208 (fluoride)
    • Fillings: D2140–D2161, D2330–D2394
    • Crowns: D2740, D2750, D2950 (build-up)
    • Root canals: D3310, D3320, D3330
    • Extractions: D7140, D7210
    • Implants: D6010, D6057, D6058/D6065
    • Night guards: D9944, D9945, D9946
  • Planned date(s) of service
  • Place of service (office)
  • Prior history the payer may ask about:
    • Date of last cleaning, exam, bitewings, and pano/FMX
    • Date of original placement for any crown being replaced
    • Date the tooth was extracted, for implant cases
    • Previous carrier, if the patient switched plans this year

Portal / eligibility check (do this first):

  • Reviewed the payer portal or Dentrix eligibility response
  • Printed or saved the portal breakdown to the patient's document center

What the portal often does NOT show, so you need to call:

  • Service history by date (last BWX, pano, prophy)
  • Exact frequency wording ("2 per calendar year" vs "1 every 6 months")
  • Waiting periods and whether they're waived
  • Missing tooth clause
  • Downgrades / alternate benefits (composite, crowns)
  • Crown/implant replacement rules (e.g., 5-year rule)
  • Night guard coverage conditions
  • Implant coverage
  • Age limits (D1120 vs D1110, fluoride, sealants)
  • History carried over from a prior carrier

2. Call Script

IVR navigation tips

  • Have the member ID, patient DOB, and practice NPI/Tax ID ready. The phone tree usually asks for them.
  • Say "representative" or "agent" clearly. If that doesn't work, try pressing 0.
  • Pick the provider option, not the member option.
  • If the system offers a fax-back or emailed benefit breakdown, request it. It can answer half your questions.
  • Note the time you dialed. Long hold times are worth logging so you learn the best call times per payer.

Opener (word for word)

"Hi, my name is [your name], calling from [practice name]. Our Tax ID is [Tax ID] and our NPI is [NPI]. I'm verifying dental benefits for a patient: [patient name], date of birth [DOB], member ID [ID]. The subscriber is [name], date of birth [DOB]. The appointment is on [date]. Can I start by confirming the plan is active?"

Clarifying questions

  • "Is that limit per calendar year, per benefit year, or once every [X] months?"
  • "Does that frequency share with another code? For example, does a pano share a limit with an FMX?"
  • "Can you give me the date of last service for that code?"
  • "Is that percentage after the deductible?"
  • "Does that apply to this patient's age?"

Asking the rep to repeat or confirm

  • "Sorry, could you repeat that slowly? I'm writing it down."
  • "Let me read that back: [repeat answer]. Is that correct?"
  • "Is that written in the plan, or is it a general guideline?"

Closing (always)

"Thank you, that's everything. Before I go, can I please have your name and the call reference number? Can you also confirm this call is documented on the member's file?"

If the rep won't give a reference number, see Section 6.

3. Core Verification Questions

# Ask Why it matters Answer
1 Is coverage active? What are the effective and termination dates? Termed plans mean the patient owes in full.
2 What is the plan type: PPO, DHMO, indemnity, Medicaid managed care? DHMOs may require an assigned office.
3 Is our office and Dr. [name] in-network for this plan? Out-of-network means higher patient cost.
4 What is the deductible (individual/family), and how much is met? Does it apply to preventive? Affects the estimate on every visit.
5 What is the annual maximum, and how much is remaining? This is the main cause of surprise crown bills.
6 Is there an out-of-pocket max? (Rare in dental; ask anyway, especially for child plans.) Some plans have one.
7 What are the coverage percentages for preventive, basic, and major? Are there copays? Needed to build the cost estimate.
8 Which category are root canals, extractions, and crowns in: basic or major? Plans categorize these differently.
9 Is the benefit period calendar year or plan year? When does it renew? Determines when the max and frequencies reset.
10 What are the frequency limits for exams, cleanings, BWX, and pano? Dates of last service? Missed frequencies lead to denied claims.

Questions

How does the insurance verification call checklist work?

You answer a few questions, pay $25, and an AI worker does the job. The result appears on your private page, usually within minutes.

Can I get changes?

Yes. Every job includes one free change request: tell the donkey what to fix and it redoes the job with your notes.

Is it really done by AI?

Yes, fully. No human writes it. AI can make mistakes, so please review the result before you rely on it.

What happens to my information?

It is used only to do your job, is never used to train AI models, and is deleted after 30 days.

What if it fails?

Failed jobs are retried once, then refunded automatically.

Need something different?

Can't find it? Describe anything.

Our AI checks it's something we can do safely, then shows you exactly what you'll get and a flat price. Nothing is charged until you order.
