Riverbend Chiropractic: Care-Plan Pricing Page & Cancellation Terms Kit
Inside: paste-ready pricing page, plain-language cancellation and refund policy, FAQ, enrollment acknowledgment form, front-desk scripts, a before/after of your current text, and a checklist of decisions and assumptions for you.
Disclaimer: This kit contains marketing copy and policy templates, not legal advice. Cancellation, refund, prepaid-package, membership/auto-renewal, and insurance rules vary by state and payer and can carry legal and licensing-board requirements. Have a licensed attorney in your state review all policy and agreement wording before you use it, and confirm insurance-related statements with your billing team or payers.
How to use this kit:
- [Brackets] mark things you need to fill in or decide. Every one of them is also listed in Part 7.
- [ASSUMPTION: …] marks places where your rule could be read more than one way. I used the clearest reading, and you should confirm it.
- The order described Parts 1 to 4 in detail. It didn't describe Parts 5 to 7, so I made these assumptions:
- Part 5 is front-desk scripts.
- Part 6 is an improved version of your current text.
- Part 7 is the assumptions-and-decisions checklist that Part 2 refers to.
- If you wanted something different, you can use your one free change request.

Clear prices, no surprises
We're a family chiropractic practice in Boise, and we think you should know exactly what you'll pay before you walk in. Most of our patients simply pay per visit, and that's always a great option. If you come in often, our care plans and membership can lower your per-visit cost, but the choice is always yours.
New patients
New patient exam + first adjustment: $89
Your first visit includes an exam and your first adjustment. Your chiropractor will discuss a care recommendation after your exam; you choose whether a plan makes sense for you. Paying per visit is always available.
Single visits and services
| Service | What's included | Price |
|---|---|---|
| New patient exam + first adjustment | Exam and your first adjustment | $89 |
| Single adjustment | One adjustment visit | $55 |
| Re-exam | [describe what a re-exam includes] | $45 |
| 30-min massage add-on | A 30-minute massage added to your visit | $40 ($35 for members) |
Care plans (optional)
Care plans let you pay ahead for a set number of adjustment visits at a lower per-visit price. Plans are never required. Single visits are always available.
| Plan | Visits | Price | Effective per-visit cost | Compared with single-visit price ($55) | Expires | Payment options |
|---|---|---|---|---|---|---|
| 12-visit care plan (paid upfront) | 12 | $540 | $540 ÷ 12 = $45.00 | $10.00 less per visit. 12 single visits = $660, so you pay $120.00 less | 6 months after purchase | One payment of $540 |
| 12-visit care plan (payment plan) | 12 | $555 total (3 × $185) | $555 ÷ 12 = $46.25 | $8.75 less per visit. $105.00 less than $660 | 6 months after purchase | 3 monthly payments of $185. Card on file required |
| 24-visit care plan (paid upfront) | 24 | $960 | $960 ÷ 24 = $40.00 | $15.00 less per visit. 24 single visits = $1,320, so you pay $360.00 less | 12 months after purchase | One payment of $960 |
Please note:
- The 12-visit payment plan totals $555. That is $15 more than paying $540 upfront.
- Unused visits must be used before the plan expires.
- If you cancel a plan, we refund the unused value. Visits you've already used are recalculated at the $55 single-visit rate. See the full policy for an example.
- Plans can be shared with immediate family members who live in your household.
- [confirm: whether re-exams and massage add-ons can be paid for with plan visits, or are always paid separately]
Wellness Membership: $119/month
- Includes: 2 adjustments per month
- Extra visits: $45 each
- Massage add-on: $35 (instead of $40)
- Renews automatically: Your membership renews every month and is charged to your card on file until you cancel. [confirm]
- To cancel: Give us 30 days' notice, any time. There is no long-term contract.
- Unused visits: Included visits don't roll over to the next month.
- A card on file is required.
Is membership right for you? Here's the honest math:
| Adjustments per month | Membership cost | Paying per visit ($55 each) |
|---|---|---|
| 1 | $119 | $55 |
| 2 | $119 | $110 |
| 3 | $119 + $45 = $164 | $165 |
| 4 | $119 + $90 = $209 | $220 |
If you come in twice a month or less, paying per visit costs less. Membership starts to make sense at 3 or more visits a month, or if you often add a massage.

- Plan expiration: 12-visit plans expire 6 months after purchase. 24-visit plans expire 12 months after purchase.
- Refunds: If you cancel a plan, we refund the unused value. Used visits are recalculated at $55 each.
